When a birth certificate is needed for any purpose such as any trip outside the United States, passport purposes, retirement, school admission or legal matters and insurance, please request it as far in advance as possible. Knowing the township in which a birth or death occurred is important because they are filed by township names. Keep in mind, when a birth or death certificate is required for any of the above reasons, you will need a certified copy. When sending for certificates, address your request to the Carroll County Health Department, 301 Moody Ave. N.W., P.O. Box 98, Carrollton, OH 44615. Include $18 for each certified copy. Personal checks and money orders are accepted. Records prior to 1908 are located at the Carroll County Genealogical Society at 24 Second St. N.E., P.O. Box 174, Carrollton, OH 44615. Business hours are Monday, Tuesday & Wednesday, 9 a.m. to noon; Thursday and Friday, 1-4 p.m. and Saturday, 10 a.m. to 2 p.m. Phone number is 330-627-9411. In an effort to preserve vital records for generations to come, the Health Department is in the process of using computer software to scan births and deaths to be stored on compact discs. The Ohio Department of Health will no longer accept calls from the Carroll County Health Department to find missing birth or death certificates. The number of calls accepted by the state is based on the number of births per county. Since Carroll County has no hospitals, very few births are recorded per year. Therefore, no calls are allowed. Prior to 1960, hundreds of births were recorded each year. Birth and death certificates can still be requested from the state but that process may be difficult when time is of the essence. |
